By purchasing a service provided by “Pure Alignment”:
I hereby give my consent to receive healing work and l acknowledge and agree that I am doing so at my own risk, while I am in “Laurens” care.
I understand that with any healing process and work on my body, my symptoms may worsen before they get better. This may be part of the healing journey.
I understand this work is designed to assist the body with healing, by helping to remove
stressors from the body.
I understand that the service provided is a Complementary healing, also known as complementary medicine, is a type of treatment that can be used in addition to, NOT instead of conventional medicine. 
I understand that healing can take time and there is no quick
immediate fix to my problem and regaining my health is a process that I must be committed to.
I have freely decided to undergo the recommended treatment and hereby give my full consent to my treatment.
I take full responsibility for my health and agree not to hold “Pure Alignment”, Lauren Endean, accountable for any medical expenses, costs caused by any health condition, known or unknown, or any mishaps which may occur whilst undergoing treatment. If I arrive late, I understand my session may be shortened to respect the timing of other appointments.

Cancellation Policy

All bookings may be rescheduled or cancelled with at least 24 hours’ notice.

Please note that all service fees are non-refundable and will be retained in cases of:

• Late cancellations (less than 24 hours’ notice)

• Missed appointments (no-shows)
